In conventional IVF, thousands of sperm are placed around each egg and one fertilises it on its own, the natural route, outside the body. Conventional IVF works well when sperm quality is good. The UK’s NHS describes IVF as one of several techniques to help a couple conceive, used after simpler options.

After collecting both eggs and sperm, the physician places them together in a laboratory culture dish, allowing the sperm to naturally penetrate the egg’s outer shell and fertilize it on its own.

This differs from ICSI, where a sperm is directly injected. Once fertilization occurs, the embryos are cultured in a specialized incubator for 3 to 5 days until they reach the appropriate stage for transfer.

What Is ICSI?

ICSI (intracytoplasmic sperm injection) is a specialised form of IVF. See the full cost breakdown and packages on the ICSI treatment page.

Generally, both IVF and ICSI procedures start from 100,000 Baht and can go up to 300,000 Baht or more. Prices vary depending on whether you choose a private hospital, a government hospital, or a specialized fertility clinic.

Note: ICSI is typically more expensive than traditional IVF. This is because ICSI involves a much more complex and delicate laboratory process requiring highly skilled embryologists and specialized micromanipulation equipment.

While ICSI offers a higher fertilization success rate, standard IVF carries a higher risk of total fertilization failure since the sperm must navigate into the egg completely on their own.

Understanding Success Rates: ICSI vs. IVF

When comparing fertilization rates, ICSI typically yields slightly higher results than IVF, with an average of about 70% of retrieved eggs successfully fertilizing. However, once fertilization is achieved, the ultimate clinical pregnancy success rates between ICSI and IVF are quite similar—generally ranging between 40% to 70%.

Success depends on various factors, including underlying fertility conditions and the physical readiness of both partners. Among these variables, maternal age is the most critical factor influencing pregnancy success:

Maternal Age

Success Rate (1st Stimulation Cycle)

Success Rate (2nd Stimulation Cycle)

30–31 years old

48%

61%

34–35 years old

40%

54%

40–41 years old

13%

21%

Ultimately, consulting a dedicated medical team at a specialized fertility clinic or hospital will provide the most accurate assessment of your specific chances of success.

Precautions and Risks

Even though ICSI yields high fertilization success rates, it requires careful handling. Because eggs are fragile, there is a minor risk of egg damage from the needle insertion. Choosing a high-standard clinic with an exceptional laboratory and experienced specialists greatly minimizes this risk. Furthermore, some research suggests comparing the risks of chromosomal abnormalities, autism, or intellectual disabilities between assisted reproductive technologies and natural conception requires careful medical evaluation, with advanced laboratory screening often helping to mitigate risks.

Frequently Asked Questions

How is ICSI different from IVF?

In IVF, sperm fertilise the egg on their own in a dish. In ICSI, a single sperm is injected directly into the egg. Everything else is the same. ICSI is chosen when sperm quality is a concern or when embryos will be genetically tested.

Is ICSI better than IVF?

Not universally. ICSI is better for male-factor infertility and cases needing PGT-A. When sperm quality is normal, conventional IVF can be equally effective, and ICSI is not routinely needed.

Is ICSI safer than IVF?

Both have similar safety profiles. ICSI adds one lab step — injecting the egg — which carries a small chance of egg damage, but no meaningful added risk to the mother. The main risks (multiple pregnancy, preterm birth) are shared by both methods and are reduced by single embryo transfer.

What are the main risks of IVF and ICSI?

The main risks are shared by both: multiple pregnancy if more than one embryo is transferred, a related chance of preterm birth or low birth weight, and a small increase in birth defects that research ties mainly to the underlying infertility rather than the technique. Overall, both are considered safe.

Does ICSI cost more than IVF?

Slightly, due to the extra embryology work. At most Bangkok clinics ICSI is the default method, so it’s priced as a package — see the ICSI in Bangkok guide for ranges.

Which should I choose — IUI, IVF or ICSI?

It depends on the cause and severity of infertility, sperm quality, and age. Milder cases may start with IUI; male-factor or more complex cases go straight to ICSI.
