Difference between IVF and ICSI (and Which One You Need)

The difference between IVF and ICSI is the fertilisation step. In conventional IVF, eggs and sperm are placed together in a dish and sperm fertilise the egg on their own. In ICSI, an embryologist injects one selected sperm directly into each egg. Both are types of in vitro fertilisation — ICSI just takes control of fertilisation, which makes it the better choice when sperm quality is a concern.

 

What Is IVF?

IVF (in vitro fertilisation) is the umbrella procedure: eggs are collected, fertilised with sperm in the laboratory, and the resulting embryo is transferred to the uterus. In conventional IVF, thousands of sperm are placed around each egg and one fertilises it on its own, the natural route, outside the body. Conventional IVF works well when sperm quality is good. The UK’s NHS describes IVF as one of several techniques to help a couple conceive, used after simpler options.

  • After collecting both eggs and sperm, the physician places them together in a laboratory culture dish, allowing the sperm to naturally penetrate the egg’s outer shell and fertilize it on its own. 
  • This differs from ICSI, where a sperm is directly injected. Once fertilization occurs, the embryos are cultured in a specialized incubator for 3 to 5 days until they reach the appropriate stage for transfer.

What Is ICSI?

ICSI (intracytoplasmic sperm injection) is a specialised form of IVF. An embryologist selects the single strongest sperm and injects it directly into the egg with a fine glass needle. Every other step — stimulation, egg collection, embryo culture, transfer — is identical to IVF. ICSI was developed specifically for male-factor infertility, and the American Society for Reproductive Medicine notes it offers little added benefit when sperm parameters are already normal.

Key Differences: ICSI vs. IVF

ICSI (Targeted Fertilization) 

  • Ideally suited for cases involving poor sperm quality or male infertility factors, such as a low sperm count, abnormal sperm shape or motility, high levels of antibodies in the semen, or a history of having a vasectomy.
  • ICSI offers a higher chance of success in these scenarios because the healthiest sperm can be selected directly for fertilization.

The ICSI Process, Explained:

The physician initiates ovarian stimulation on the 2nd day of the menstrual cycle, administering daily injections for 8–14 days.

    • An ultrasound monitors the size and number of follicles.
    • Once ready, an ovulation-inducing trigger shot is given.
    • The eggs are retrieved through the vagina under ultrasound guidance.
    • The eggs and sperm are then fertilized in the lab while the woman receives hormonal medication to prepare her uterine lining.
    • Once the embryos are strong and viable, they are transferred into the uterine cavity.

IVF (Natural Lab MIxing )

  • Best suited for women aged 37 or younger (particularly under 30) who have damaged or blocked fallopian tubes, or ovulation disorders, but where the male partner’s sperm quality is normal.

The IVF Process, Explained:

    • The physician assesses readiness before starting ovarian stimulation around day 2 or 3 of the menstrual cycle, followed by abdominal injections for 8–14 days.
    • When the eggs are mature, a trigger shot is given, and egg retrieval is scheduled.
    • The male partner provides a semen sample on the same day.
    • After fertilization and culturing for 3–5 days, the embryos are transferred. This can be done in a fresh cycle (3–5 days after retrieval) or a frozen cycle, where the woman’s uterine lining is prepared separately, and thawed embryos are transferred once the uterus is perfectly ready.
    • A blood test to check pregnancy hormone levels is scheduled about 10 days after the transfer

When Is ICSI Better Than IVF?

ICSI is the better choice when leaving fertilisation to chance is risky. Consider ICSI if:

  • Sperm count is low — below roughly 15 million per millilitre, under the World Health Organization reference range — or motility/shape is poor.
  • There’s no sperm in the ejaculate (azoospermia), where sperm is retrieved surgically (TESE).
  • A previous IVF cycle had poor or failed fertilisation.
  • Embryos will be genetically tested with PGT-A, which pairs naturally with ICSI and a later frozen embryo transfer (FET).
  • Recurrent miscarriage or advanced maternal age, often alongside an AMH test to check ovarian reserve.

If sperm quality is normal and the issue is tubal or unexplained, conventional IVF may be equally effective — a specialist assessment decides. The UK’s NICE fertility guideline similarly reserves ICSI for cases with a clear indication rather than using it by default.

ICSI vs IUI: Which Should You Choose?

People often weigh ICSI against IUI (intrauterine insemination) too. IUI is simpler and cheaper but has a lower success rate per cycle, so doctors often try it first for milder cases. Where there’s a clear indication — blocked tubes or very low sperm count — ICSI may be recommended from the start.

Does ICSI Have a Higher Success Rate Than IVF?

For couples with a male-factor problem, yes.

Because ICSI guarantees a healthy sperm reaches the egg, its fertilisation rate is higher and more predictable than conventional IVF. Where sperm is normal, the two are broadly comparable, and international bodies such as ESHRE caution against using ICSI routinely without an indication. Are There Risks or Downsides to IVF and ICSI?

  • Multiple pregnancy (twins or triplets) if more than one embryo is transferred, or from a strong response to stimulation. Twin pregnancies carry higher risks, which is why GFC’s default is single embryo transfer.
  • A higher chance of preterm birth or low birth weight, mainly linked to multiple pregnancy.
  • A small increase in birth defects compared with natural conception. Importantly, research links this largely to the underlying infertility itself, not to the ICSI or IVF technique — the Human Fertilisation & Embryology Authority (HFEA) and NHS both note the absolute risk remains low.
  • The emotional and physical demands of a treatment cycle, which are real and worth planning for.

One consideration specific to ICSI: because the egg is injected with a fine needle, there’s a small chance of egg damage during the procedure — a well-equipped lab and experienced embryology team keep this to a minimum.

(Note: contrary to some claims online, ICSI does not reduce the risk of chromosomal abnormalities or autism — chromosomal screening is done separately with PGT-A. Ask your specialist for an evidence-based picture of your own risks.)

Even though ICSI yields high fertilization success rates, it requires careful handling. Because eggs are fragile, there is a minor risk of egg damage from the needle insertion. Choosing a high-standard clinic with an exceptional laboratory and experienced specialists greatly minimizes this risk. Furthermore, some research suggests comparing the risks of chromosomal abnormalities, autism, or intellectual disabilities between assisted reproductive technologies and natural conception requires careful medical evaluation, with advanced laboratory screening often helping to mitigate risks.
